Q. WHAT SERVICES ARE PROVIDED AT ST. LUKE'S HOSPITAL?

A. St. Luke's Hospital is a full tertiary hospital. This includes a fully functional Emergency Department, Surgical Services, Inpatient units, and Radiology Services. Surgical Services has specialized service lines in Cardiovascular, Neurology, Gynecological, Orthopedic, and General procedures. Surgical Scheduling can be reached at 904-296-5600. Outpatient Radiology also has state of the art Digital Mammography, Invasive stereotactic biopsy, and Ultrasound. Additional programs at St.Luke's Hospital include Epilepsy Monitoring, Bariatric Surgical Education, and Diabetes Education.

Q. I WAS A PATIENT AT ST. LUKE'S HOSPITAL BEFORE APRIL 12, 2008. HOW CAN I GET MY MEDICAL RECORDS?

A. All Medical Records of patients prior to April 12, 2008 are with Mayo Clinic. Please contact Mayo Clinic directly by calling 904-953-2022 with inquiries. All Medical Records from April 12, 2008 forward are kept at St. Luke's Hospital and can be obtained by calling 904-296-3758.

Q. IS THERE OUTPATIENT LABORATORY SERVICES LOCATED AT ST. LUKE'S HOSPITAL?

A. Yes, Consolidated Laboratory Services is located on the first floor of the Roger Main building. Consolidated Laboratory is open 7 a.m. to 4 p.m., Monday through Friday. The phone number there is 904-296-3811.

Q. HOW DO I GET TO THE FAMILY BIRTH PLACE?

A. There is a separate entrance to the Family Birth Place to the North of the St. Luke's Emergency Room Entrance.

Once on campus, follow the directional signs to a private parking lot and entrance.

Q. WHERE CAN I ADDRESS CONCERNS AND COMMENTS ABOUT ST. LUKE'S HOSPITAL?

A. St. Luke's Hospital is committed to providng excellent care to all of our patients and visitors. We encourage concerns to be addressed with Management or Administration while in our facilities so we can improve on any issues brought to our attention. Also, throughout the hospital, you will find Comment Cards, which can be completed and left in the secure box or mailed with prepaid postage. Additionally, the Risk Management Department can be contact at 904-308-7350.
